I am first and foremost a practicing family physician, with an active part-time practice in Ann Arbor for more than 30 years. My academic work has focused on developing the human and technical infrastructure needed to support integrated primary health care for many years. At the University of Michigan we designed, implemented, and licensed clinical decision support software (ClinfoTracker) and developed a primary care depression care support intervention and its supporting IT platform (M-DOCC Clinical Record) eventually used in several academic and community primary care practices in Michigan.

For the past several years, I’ve worked directly with communities to develop ‘community information exchanges’ to support integrated health and social care delivery. We have developed a community co-design process to reduce power disparities that undermine collaborations. Though the State Innovation model in MI, I helped to co-create a shared care model and a simple, scalable technology infrastructure to support cross-sector care which has served as a model for other communities to create ‘social provider organizations’ which can make cross-sector care possible.

Since January 2020, I have served as a consultant and most recently the SDOH Program Medical Director for the Michigan Health Information Network (MIHIN,) working collaboratively to develop a statewide approach towards infrastructure that supports cross-sector care. In this work we have leaned on national collaborations like Gravity, DaVinci, and Project Unify to provide conceptual and technical foundations on which simple data exchange tools can be flexibly deployed.
